Clinical Case-Study Submission Criteria
Submission Deadline December 21, 2009
Selected cases will be presented for Interactive Discussion
General Format Specifications:
- Describe only the most pertinent sequence of events, physical examination and the patient's progress and outcome. Be succinct and complete. Eliminate non-essential elements of the case.
- Intervention: State why decisions were made and extract the lesson from the case. Reports from the literature, or their absence should be cited.
- Submit the ONE Page the vignette in Microsoft Word format in 12 point Times New Roman font.
- All text is single spaced.
- Margins are left justified.
- A single space is used after the period at the end of a sentence.
- Do not include patient identifying information such name, date of birth or medical record number.

Case Study Line 1 - Title:
- Title is in upper and lower case letters with the proper words capitalized.
- Title is left justified.
- Title is in bold.
- One blank space occurs between the title and Line 2.
Case Study Line 2:
- Line 2 consists of First name, middle initial (with period), and last name of author followed by any co-authors.
- All authors names are italicized;
